Three-Year Outcomes After Bifurcation Stenting With Zotarolimus-Eluting Stents: Final Results From the RESOLUTE ONYX Postapproval Study

Background: Bifurcation represents a challenging lesion subset for percutaneous coronary intervention. Methods: In this prospective study of the Resolute Onyx zotarolimus-eluting stent (ZES), patients with a single bifurcation target lesion who underwent planned treatment using a provisional stenting technique were enrolled at 25 centers in the United States and Europe. The primary end point was target-vessel failure (TVF) at 1 year, and follow-up was performed through 3 years. Results: A total of 205 patients were enrolled. Mean age was 66.6 ± 10.7 years, 21.5% of patients were female, and diabetes mellitus was present in 30.2%. A provisional approach with a single stent was performed in 96.6% of patients. The rate of TVF at 1 year was 7.4%, fulfilling the prespecified performance criterion (upper 1-sided 95% CI of 11.1%, compared with the performance goal of 24.5%). At 3-year follow-up, the rate of TVF was 12.1%, the rate of clinically driven target-lesion revascularization was 6.0%, and there were no episodes of stent thrombosis related to the target lesion. Event rates were consistent among the cohort of patients with angiographic core laboratory-confirmed bifurcation lesions. Conclusions: In this prospective, multicenter study, bifurcation lesion treatment with Resolute Onyx ZES using a planned provisional stent approach was associated with favorable clinical outcomes through 3 years. These results support the longer-term safety and effectiveness of Resolute Onyx ZES to treat bifurcation lesions that are amenable to a planned provisional stenting technique.

Association between overcrowded households, multigenerational households, and COVID-19: a cohort study

OBJECTIVES/OBJECTIVE:The role of overcrowded and multigenerational households as a risk factor for COVID-19 remains unmeasured. The objective of this study is to examine and quantify the association between overcrowded and multigenerational households and COVID-19 in New York City (NYC). STUDY DESIGN/METHODS:Cohort study. METHODS:We conducted a Bayesian ecological time series analysis at the ZIP Code Tabulation Area (ZCTA) level in NYC to assess whether ZCTAs with higher proportions of overcrowded (defined as the proportion of the estimated number of housing units with more than one occupant per room) and multigenerational households (defined as the estimated percentage of residences occupied by a grandparent and a grandchild less than 18 years of age) were independently associated with higher suspected COVID-19 case rates (from NYC Department of Health Syndromic Surveillance data for March 1 to 30, 2020). Our main measure was an adjusted incidence rate ratio (IRR) of suspected COVID-19 cases per 10,000 population. Our final model controlled for ZCTA-level sociodemographic factors (median income, poverty status, White race, essential workers), the prevalence of clinical conditions related to COVID-19 severity (obesity, hypertension, coronary heart disease, diabetes, asthma, smoking status, and chronic obstructive pulmonary disease), and spatial clustering. RESULTS: = 0.99, 95% CI: 0.99-1.00). CONCLUSIONS:Overcrowdedness and multigenerational housing are independent risk factors for suspected COVID-19. In the early phase of the surge in COVID cases, social distancing measures that increase house-bound populations may inadvertently but temporarily increase SARS-CoV-2 transmission risk and COVID-19 disease in these populations.

BACKGROUND:The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. METHODS:This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. FINDINGS:This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28-2·40], p<0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65-3·22], p<0·0001), American Society of Anesthesiologists grades 3-5 versus grades 1-2 (2·35 [1·57-3·53], p<0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01-2·39], p=0·046), emergency versus elective surgery (1·67 [1·06-2·63], p=0·026), and major versus minor surgery (1·52 [1·01-2·31], p=0·047). INTERPRETATION:Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery. Collective vulnerability is associated with higher perceived discrimination, pain, and depression in veterans with osteoarthritis [Meeting Abstract]

Background: Perceived discrimination is associated with physical and mental health outcomes, including chronic pain and depression, and plays a role in racial health disparities. However, little is known about how the intersection of race and other factors for which one might experience discrimination (collective vulnerability), such as gender, education, income, weight, or disability, impact health. Our aim was to examine the additive effects of six vulnerable group memberships on perceived discrimination, pain, and depression and to test whether perceived discrimination mediates the impact of collective vulnerability on pain and depression.
Method(s): Our sample included patients at two large VA medical centers who enrolled in a randomized controlled trial testing a positive psychological intervention for chronic pain. Participants were white and African American Veterans aged 50 years reporting symptomatic knee osteoarthritis (OA). At baseline, we measured perceived discrimination using the Everyday Discrimination Scale; severity of pain using the pain subscale of the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC); depression severity using the Personal Health Questionnaire Depression Scale (PHQ-8); and demographic variables (self-reported gender, age, race, income, education, and employment). We abstracted body mass index (BMI) from medical records. We assigned binary indicators of vulnerability to participants with each of the following characteristics: African American race, female gender, high school education or less, < $ 20,000 income, disabled employment status, and morbid obesity (BMI> 40). We then summed individual indicators to capture each participant's number of vulnerabilities. We conducted separate linear regression models to assess the relationship between the number of vulnerabilities and measures of perceived discrimination, pain, and depression. Lastly, we used Sobel's test of mediation to examine perceived discrimination as a mediator of the relationships between collective vulnerability and pain and depression.
Result(s): Among the 517 participants (47.8% white, 52.2% African American), the average number of vulnerabilities was 1.66 (SD=1.17). Number of vulnerabilities was significantly associated with perceived discrimination (Coef = 0.41, 95% CI 0.18, 0.65), pain (Coef = 4.14, 95% CI 2.92, 5.37) and depression severity (Coef = 1.13, 95% CI 0.74, 1.51). Although perceived discrimination significantly explained 16.9% of the association between vulnerabilities and depression (p-value < 0.01), it did not mediate the association with pain (p-value =0.10).
Conclusion(s): Among Veterans with chronic pain from OA, collective vulnerability is associated with higher levels of perceived discrimination, pain, and depression. Perceived discrimination also partially mediates the relationship between vulnerabilities and depression. Further understanding of how vulnerabilities combine to worsen physical and mental health outcomes is warranted

Analysis of TET2 mutations in paroxysmal nocturnal hemoglobinuria (PNH)

Background/UNASSIGNED:as a candidate gene in which mutations might be contributing to clonal expansion. Methods/UNASSIGNED:genes in 19 patients with large PNH clones. Results/UNASSIGNED:in multiple hematopoietic lineages, which was detectable upon repeat testing. This patient has had severe thromboses and has relatively higher peripheral blood counts compared with the other patients-but does not have other features of a myeloproliferative neoplasm. Conclusions/UNASSIGNED:may contribute to clonal expansion in exceptional cases of PNH.

C-reactive protein and disease activity in rheumatoid arthritis: Impact of obesity and adiposity [Meeting Abstract]

Background/Purpose: C-reactive protein (CRP) is used to assess disease activity in rheumatoid arthritis (RA), and previous work suggests that adiposity also impacts CRP levels. This study assessed associations between BMI and CRP, hypothesizing that increasing levels of obesity are associated with elevations of CRP independent of RA disease activity, paralleling what is seen in the general population. Methods: Associations between BMI and CRP were assessed in two RA cohorts - 1) a cross-sectional Body Composition (BC) cohort (N = 451) pooled from 3 independent studies from US academic centers that included wholebody DXA measures of fat mass index, and 2) the longitudinal Veterans Affairs Rheumatoid Arthritis (VARA) registry (N = 1652). For comparison, associations were also evaluated in the general population using data from the National Health and Nutrition Examination Survey (NHANES) 2007-2010 (N = 10,813). Linear and logistic regression analyses (defining high CRP as CRP > 1.0 mg/dL and using generalized estimating equations to incorporate repeated measures in VARA) were stratified by sex and adjusted for age, race, and smoking. Sequential models assessed the impact of adjustment for disease activity (swollen/tender joints, patient global score), and fat mass in BC only as this measure was not available for VARA. Results: In all three cohorts (NHANES, VARA, BC), women in higher BMI categories had significantly higher CRP (all p < 0.001; BMI > 35 vs 20-25 kg/m2) (Figure). This association remained after adjusting for joint counts and patient global scores (p < 0.001 in BC; p < 0.01 in VARA) but was completely attenuated when adjusted for fat mass in BC. Women with BMI > 35 kg/m2 were also more likely to have an elevated CRP (Table 1). Again associations remained after adjusting for disease activity (BC: OR 4.05, p = 0.02; VARA: OR 2.82, p = 0.01), but were attenuated with adjustment for fat mass in the BC cohort (OR 0.99, p = 1). Among men, BMI > 35 (vs BMI 20-25 kg/m2) was not associated with higher CRP in VARA or BC (Figure). In VARA, but not in controls, men with low BMI <20 kg/m2 had higher CRP and greater odds of an elevated CRP (all p < 0.01). In BC, fat mass index was associated with a greater odds of an elevated CRP in women (OR 1.69 per standard deviation, p < 0.01) but a lower odds in men (OR 0.60, p < 0.01). Conclusion: Morbid obesity is associated with greater CRP in women with RA, similar to what is seen in the general population. This association is related to fat mass and not RA disease activity, necessitating caution when interpreting CRP among women with a high BMI. Causes of high CRP in low BMI men with RA require further study. (Table presented)

Limited miR-17-92 overexpression drives hematologic malignancies

The overexpression of microRNA cluster miR-17-92 has been implicated in development of solid tumors and hematological malignancies. The role of miR-17-92 in lymphomagenesis has been extensively investigated; however, because of the developmental defects caused by miR-17-92 dysregulation, its ability to drive tumorigenesis has remained undetermined until recently. Here we demonstrate that overexpression of miR-17-92 in a limited number of hematopoietic cells is sufficient to cause B cell malignancies. In sum, our study provides a novel and physiologically relevant model that exposes the potent ability of miR-17-92 to act as a driver of tumorigenesis.

In Vivo mapping of notch pathway activity in normal and stress hematopoiesis

Accumulating evidence suggests that Notch signaling is active at multiple points during hematopoiesis. Until recently, the majority of such studies focused on Notch signaling in lymphocyte differentiation and knowledge of individual Notch receptor roles has been limited due to a paucity of genetic tools available. In this manuscript we generate and describe animal models to identify and fate-map stem and progenitor cells expressing each Notch receptor, delineate Notch pathway activation, and perform in vivo gain- and loss-of-function studies dissecting Notch signaling in early hematopoiesis. These models provide comprehensive genetic maps of lineage-specific Notch receptor expression and activation in hematopoietic stem and progenitor cells.
