Pulmonary embolization of iodine-125 seeds following prostate implantation

The optimal treatment of prostatic carcinoma limited to the gland remains controversial. Treatment has included implantation of Iodine-125 seeds via both a suprapubic approach and, more recently, a transperineal technique utilizing ultrasound guidance. We recently have noted a heretofore unreported complication with this latter technique, namely, embolization of seeds to the lungs. Review of the chest x-ray films of 31 patients who underwent suprapubic implants showed no evidence of this phenomenon. One of 5 patients undergoing transperineal implant was found to have seeds lodged in the lung. Complications surrounding the various treatments of localized prostate carcinoma are reviewed

Solitary metastasis of renal cell carcinoma to urethra [Case Report]

A case report of a solitary metastasis to the urethra from previously resected renal cell carcinoma is reported. The patient presented with total gross painless hematuria. We believe this is the first such case to be reported.

Angiosarcoma of kidney [Case Report]

We describe a forty-six-year-old man who died of disseminated angiosarcoma ten months after a radical nephrectomy and postoperative radiation therapy for a renal angiosarcoma. We found 3 other well-documented reports of renal angiosarcoma, all in men. Two of the 3 cases previously reported also had a rapidly fatal course with hematogenous spread of the neoplasm following nephrectomy. Angiosarcoma of the kidney, although rare, is apparently a highly malignant fatal neoplasm, and effective therapy remains unknown.

Renal angiosarcoma [Case Report]

Renal angiosarcoma, an uncommon vascular sarcoma, is primarily treated surgically. We present a patient who, at nephrectomy, had tumor adherent to the diaphragm and renal artery. Postoperative radiation was given to the renal fossa. Despite the subsequent development of bone and liver metastases, disease in the renal fossa was apparently controlled. Symptomatic bone lesions were effectively palliated with radiotherapy. This report confirms those of others suggesting that angiosarcoma does respond to radiation

The role of internal spermatic vein plasma catecholamine determinations in subfertile men with varicoceles

Catecholamine concentrations were determined in left spermatic vein blood of 18 patients with varicocele. These values were compared with concentrations of catecholamines in the right femoral vein. Of 18 patients studied, 6 had moderate to marked elevations of spermatic vein catecholamine concentrations. Norepinephrine was the fractioned component that was elevated. In an average 1-year follow-up the group with elevated catecholamine levels had a much better semen improvement percentage and pregnancy rate than the other group. Chronic exposure of testicular vasculature to norepinephrine may account for the spermatogenic dysfunction encountered in varicocele patients. Once this exposure is interrupted, there may be reversibility of function and the fertility prognosis may improve. The presence of other adrenal or renal metabolites detrimental to spermatogenesis must be considered
