Closing the gaps: the effectiveness of linking programs connecting chronic mental patients from the hospital to the community

The research reported in this article tested the effectiveness of a linking program between city hospitals and community treatment programs in reducing the rehospitalization of mental patients following discharge. The authors proposed that the nature of the relationships between the liaison teams of such programs and those providing aftercare services is essential to providing a continuity of care and thus to lowering rates of rehospitalization. Two liaison teams were evaluated, one of which had more positive and ongoing relationships with providers of community services to mental patients. The program with the more positive and continuous relationships was predicted and found to be more effective in reducing rehospitalization rates for chronic mental patients. Testing a case management/primary therapist adaptation by the less effective program demonstrated that patient or staff characteristics did not account for the differences between the programs' effectiveness

Achievement motivation in medical students

Medical students were assessed by questionnaire in their third and fourth years regarding attitudes about sex role, medical school, and career plans and in the third year about fear of success. Nine percent of each sex were judged to fear success. Stereotypical responses were reported by the students. Males, anticipating that they would work more than 60 hours per week, looked forward to academic careers, while females more often desired the sharing of financial and child care responsibilities with spouses. Attitudes toward chores were egalitarian, but in reality women performed most routine household activities. Both sexes agreed upon the characteristics that are necessary to be a good student as well as to succeed in later life. Women were more inclined toward artistic and domestic interests and helping others, while men were more interested in scientific investigation, athletics, and adventure. The implications of these similarities and differences for the students' future careers are discussed

Elucidates the problems of the psychiatric patient in a large metropolitan area that has attempted to deinstitutionalize. The major difficulties are summarized: (1) failure of the catchment system to provide uniformly good care for all, (2) reduction of the quantity and quality of services because of diverse budget constraints, (3) inequitable and overly narrow reimbursement procedures affecting public hospitals, (4) lack of coordinated programs for dealing with the elderly and the chronically ill who are without relatives and friends, (5) recent increase in problems in living facing the psychiatric patient, and (6) lack of easy access to inpatient and outpatient services by the middle class and working poor.

Transfers to a psychiatric emergency room: a fresh look at the dumping symdrome

There can be considerable interinstitutional distrust regarding transfer of psychiatric patients. In 1970 the directors of all psychiatric hospitals in New York City began to meet regularly in order to define problems and work out amicable resolutions, largely by defining criteria for hospitalization, establishing correct catchment area address, etc. In order to test the success of the program, three medical centers which routinely transfer patients to a receiving hospital were studied in order to see if they misrepresented patients' psychopathology or medical problems prior to transfer. It was found that 90% of the transfers were warranted, but that preselection at the hospital or origin led to the transfer of more males than females and more assaultive, rather than quieter patients, who might have been more amenable to inpatient treatment

Charge to the community: Some early effects of a state hospital system's change of policy

Cites the current thrust toward community care in New York State, which has had a dramatic positive effect as local facilities have actively presented alternatives to long-term hospitalization. However, there have also been negative effects-some anticipated and some unanticipated-as the same local facilities have found it difficult to meet the needs of patients unable to make the transition from custodial care to living on their own. These developments are discussed in light of New York City's recent experiences with community psychiatry, and some recommendations are offered for similar transitions in other communities.

Re-examines the generalizability of conclusions by M. Horner (see record 1973-09174-001) that women show significant and increasing avoidance of the motive to achieve success. Results on 101 male and female Ss with controls on raters (n = 9) found contrary results. Overgeneralizations based on a limited data base and the basis on which the concept of success is determined are questioned.
