Stacks Repair and Preservation Request Form

The Ehrman Medical Library needs your help in finding and identifying repairs and preservation problems with the book and journal stack areas located downstairs of the library’s main circulation floor.

Please leave us as much information as possible as to what the repair or preservation issue there is (leaks, more bookends, floor-tiles loose, complaints of food, etc.) and we will address the matter.

The Stacks Repair and Preservation Request Form is designed to open communications between patron and library staff about the overall appearance of the stacks. This is your opportunity to help us have a better more efficient library for
you-the patron.

Leaks:
(Where the leak is located...between what range numbers)

Bookends:
(bookends are needed in what section---write the call number(s), aisle range)

Tile Repair:
(missing, cracked, or loose tiles)

Other:
(other repairs, people eating food, etc.)
