Through the description of the process of psychotherapy of an obsessional patient by a psychiatric resident under supervision a number of factors which impact on treatment response and outcome are focused upon. The aim is to elicit the multiple factors impinging upon a single treatment situation with some broader implications for research as well as training

Argues that there is a danger that the central role of doing psychotherapy in becoming a psychiatrist has been and will continue to be downgraded. It is suggested that learning to do psychotherapy makes core contributions to the development of a psychiatrist in 3 overlapping areas essential for functioning: doctor/patient relationships, the development of a psychiatric identity, and the understanding of mental illness.
