High temporal resolution 3D gadolinium-enhanced dynamic MR imaging of renal tumors with pharmacokinetic modeling: Preliminary observations

PURPOSE: To assess dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI) tracer pharmacokinetic parameters obtained with Generalized Kinetic Model (GKM) and extended Shutter Speed Model (SSM2) in renal tumors stratified by histologic subtypes. MATERIALS AND METHODS: In all, 24 patients with renal tumors were imaged at 1.5 T utilizing DCE-MRI with high temporal resolution (1.2 sec/temporal frame) prior to surgery. Tracer kinetic analysis was performed for the entire tumor using individualized aortic input function. GKM and SSM2 were employed to generate transfer constant (K(trans) ), plasma volume, and interstitial volume. These parameters, and DeltaK(trans) (K(trans) SSM2 - K(trans) GKM) were compared between tumors stratified by histologic subtype. RESULTS: There were 25 renal tumors: 15 clear cell, 4 papillary, 3 chromophobe, and 3 oncocytoma/oncocytic subtype. K(trans) GKM was significantly higher in chromophobe compared to other subtypes (P < 0.01). Using K(trans) GKM > 1.0 min(-1) , chromophobe were diagnosed with 100% sensitivity and 90.9% specificity. K(trans) SSM2 was higher than K(trans) GKM for all renal tumors except for all chromophobe and two clear cell subtype. Using K(trans) GKM > 1.0 min(-1) and Delta K(trans) < 0, chromophobe could be discriminated from other lesions with 100% accuracy. CONCLUSION: K(trans) obtained with GKM and SSM2 analysis can potentially discriminate chromophobe from other renal lesions with high accuracy. J. Magn. Reson. Imaging 2013;. Prostate Cancer Diagnosis Among Men With Isolated High-Grade Intraepithelial Neoplasia Enrolled Onto a 3-Year Prospective Phase III Clinical Trial of Oral Toremifene

PURPOSE Prostate cancer (PCa) prevention remains an appealing strategy for the reduction of overtreatment and secondary adverse effects. We evaluated the efficacy of toremifene citrate 20 mg in PCa prevention among men with isolated high-grade prostatic intraepithelial neoplasia (HGPIN) on biopsy. PATIENTS AND METHODS One thousand five hundred ninety men with HGPIN, or HGPIN and atypia, and no PCa on prostate biopsy were randomly assigned 1:1 to receive toremifene citrate 20 mg or placebo in a 3-year phase III, double-blind, multicenter trial. Men underwent annual biopsy until cancer detection or study end. Efficacy analysis was performed in 1,467 men who underwent at least one on-study biopsy. Baseline risk factors were evaluated to determine influence on cancer detection. Results Cancer was detected in 34.7% and 32.3% of men in the placebo and treatment groups, respectively, with no observed difference (P = .39, log-rank test) in PCa-free survival. The 3-year Kaplan-Meier PCa-free survival estimate was 54.9% (99% CI, 43.3% to 66.5%) in the placebo group and 59.5% (99% CI, 48.1% to 70.9%) in the treatment group. Exploration of baseline risk factors demonstrated no subset in which a risk reduction was observed. In the placebo group, 17.9%, 12.9%, and 13.6% of men at risk at the beginning of years 1, 2, and 3, respectively, were diagnosed with PCa. CONCLUSION Although toremifene 20 mg did not lower the PCa detection rate, men with isolated HGPIN have a high likelihood of eventual PCa diagnosis, demonstrating they are ideal candidates for inclusion in chemoprevention trials and require surveillance by periodic prostate biopsy.

Diffusion-weighted imaging of the prostate: Comparison of b1000 and b2000 image sets for index lesion detection

PURPOSE: To compare tumor detection on acquired diffusion-weighted (DW) images and apparent diffusion coefficient (ADC) maps, obtained using b-values of 1000 s/mm(2) and 2000 s/mm(2) , using radical prostatectomy as the reference. MATERIALS AND METHODS: In all, 29 prostate cancer patients who underwent 3T magnetic resonance imaging (MRI) including DW imaging using b-values of 1000 s/mm(2) and 2000 s/mm(2) were included. Two radiologists independently evaluated four image sets during different sessions and recorded the location and diameter of the dominant lesion: DW images acquired using b-values of 1000 s/mm(2) and 2000 s/mm(2) and ADC maps calculated using maximal b-values of 1000 s/mm(2) and 2000 s/mm(2) . Findings were correlated with the location and diameter of the dominant lesion at prostatectomy. Tumor-to-PZ contrast was also calculated, unblinded to pathology. RESULTS: Both readers achieved significantly higher sensitivity for DW images obtained using a b-value of 2000 s/mm(2) than 1000 s/mm(2) (P < 0.001), although there was no difference in sensitivity between ADC maps calculated using the two b-values (P >/= 0.309). Tumor-to-PZ contrast was higher for DW images using a b-value of 2000 s/mm(2) (P = 0.067), although it was not different between the two corresponding ADC maps (P = 0.544). For both readers, correlations with tumor diameters were higher for either ADC map (r = 0.59-0.73) than for either acquired DW image set (r = 0.03-0.57). CONCLUSION: Use of a b-value of 2000 s/mm(2) compared with a b-value of 1000 s/mm(2) resulted in improved tumor sensitivity and higher tumor-to-PZ contrast on the acquired DW images, although performance of the ADC maps corresponding with the two b-values was similar. Prostate cancer: Utility of diffusion-weighted imaging as a marker of side-specific risk of extracapsular extension

PURPOSE: To assess the utility of diffusion-weighted imaging (DWI) findings as an indirect marker of side-specific risk of extracapsular extension (ECE) of prostate cancer. MATERIALS AND METHODS: Fifty-one patients underwent 3T magnetic resonance imaging (MRI) before prostatectomy. Radiologists 1 and 2 (4 and 1 years experience) assessed each side for ECE using T2-weighted imaging (T2WI) and evaluated apparent diffusion coefficient (ADC) maps for the presence of apparent tumor in each lobe and to measure peripheral zone ADC. A uropathologist measured the extent of any ECE. RESULTS: In all, 28/102 lobes had ECE, of which 12 measured =1 mm, 11 measured >1 mm and =2 mm, and five measured >2 mm. Side-specific accuracies for detection of ECE for readers 1 and 2 were respectively: T2WI 68.6% and 74.5%; presence of apparent tumor on ADC map 66.7% and 60.8%; ADC value 75.5% and 69.6%. For ECE >2 mm, both readers achieved 100% sensitivity based on apparent tumor on ADC map or ADC values and 80% sensitivity using T2WI. For detection of ECE =2 mm, sensitivity for all combinations of the three methods and two readers ranged from 58.3%-81.8%, aside from assessment for ECE using T2WI by the less experienced reader, which exhibited sensitivity of 17.4%. Interreader agreement for the presence of ECE was 0.18 using T2WI, 0.37 using apparent tumor on ADC map, and 0.60 using ADC values. CONCLUSION: Compared with T2WI, DWI had comparable accuracy for side-specific assessment of ECE, greater sensitivity for ECE <2 mm for the less-experienced radiologist, and greater interreader agreement. Image-guided prostate biopsy using magnetic resonance imaging-derived targets: a systematic review

CONTEXT: Technical improvements in prostate magnetic resonance imaging (MRI) have resulted in the use of MRI to target prostate biopsies. OBJECTIVE: To systematically review the literature to compare the accuracy of MRI-targeted biopsy with standard transrectal biopsy in the detection of clinically significant prostate cancer. EVIDENCE ACQUISITION: The PubMed, Embase, and Cochrane databases were searched from inception until December 3, 2011, using the search criteria 'prostate OR prostate cancer'AND 'magnetic resonance imaging OR MRI'AND 'biopsy OR target'. Four reviewers independently assessed 4222 records; 222 records required full review. Fifty unique records (corresponding to 16 discrete patient populations) directly compared an MRI-targeted with a standard transrectal approach. EVIDENCE SYNTHESIS: Evidence synthesis was used to address specific questions. Where MRI was applied to all biopsy-naive men, 62% (374 of 599) had MRI abnormalities. When subjected to a targeted biopsy, 66% (248 of 374) had prostate cancer detected. Both targeted and standard biopsy detected clinically significant cancer in 43% (236 or 237 of 555, respectively). Missed clinically significant cancers occurred in 13 men using targeted biopsy and 12 using a standard approach. Targeted biopsy was more efficient. A third fewer men were biopsied overall. Those who had biopsy required a mean of 3.8 targeted cores compared with 12 standard cores. A targeted approach avoided the diagnosis of clinically insignificant cancer in 53 of 555 (10%) of the presenting population. CONCLUSIONS: MRI-guided biopsy detects clinically significant prostate cancer in an equivalent number of men versus standard biopsy. This is achieved using fewer biopsies in fewer men, with a reduction in the diagnosis of clinically insignificant cancer. Variability in study methodology limits the strength of recommendation that can be made. There is a need for a robust multicentre trial of targeted biopsies.

Phosphorylation of the androgen receptor by PIM1 in hormone refractory prostate cancer

Integration of cellular signaling pathways with androgen receptor (AR) signaling can be achieved through phosphorylation of AR by cellular kinases. However, the kinases responsible for phosphorylating the AR at numerous sites and the functional consequences of AR phosphorylation are only partially understood. Bioinformatic analysis revealed AR serine 213 (S213) as a putative substrate for PIM1, a kinase overexpressed in prostate cancer. Therefore, phosphorylation of AR serine 213 by PIM1 was examined using a phosphorylation site-specific antibody. Wild-type PIM1, but not catalytically inactive PIM1, specifically phosphorylated AR but not an AR serine-to-alanine mutant (S213A). In vitro kinase assays confirmed that PIM1 can phosphorylate AR S213 in a ligand-independent manner and cell type-specific phosphorylation was observed in prostate cancer cell lines. Upon PIM1 overexpression, AR phosphorylation was observed in the absence of hormone and was further increased in the presence of hormone in LNCaP, LNCaP-abl and VCaP cells. Moreover, phosphorylation of AR was reduced in the presence of PIM kinase inhibitors. An examination of AR-mediated transcription showed that reporter gene activity was reduced in the presence of PIM1 and wild-type AR, but not S213A mutant AR. Androgen-mediated transcription of endogenous PSA, Nkx3.1 and IGFBP5 was also decreased in the presence of PIM1, whereas IL6, cyclin A1 and caveolin 2 were increased. Immunohistochemical analysis of prostate cancer tissue microarrays showed significant P-AR S213 expression that was associated with hormone refractory prostate cancers, likely identifying cells with catalytically active PIM1. In addition, prostate cancers expressing a high level of P-AR S213 were twice as likely to be from biochemically recurrent cancers. Thus, AR phosphorylation by PIM1 at S213 impacts gene transcription and is highly prevalent in aggressive prostate cancer.Oncogene advance online publication, 17 September 2012; doi:10.1038/onc.2012.412.
