MRI quantitative morphologic analysis of patellofemoral region: lack of correlation with chondromalacia patellae at surgery

OBJECTIVE: In numerous studies, the morphologic features of the patellofemoral joint have been analyzed on radiographs. The objective of this study was to assess patellofemoral measurements on MR images and to correlate the measurements with the presence or absence of chondromalacia patellae confirmed at surgery. MATERIALS AND METHODS: Axial and sagittal MR images of 98 knees (97 patients) were evaluated. Lateral and medial patellar facet lengths, lateral-to-medial facet length ratio, and interfacet angle were measured at three levels through the patella. Trochlear depth was measured on an axial slice. Patella and patellar tendon lengths, patellar tendon-to-patella ratio, and overlap of the patellar and trochlear articular cartilages were measured on sagittal slices. These measurements in knees with chondromalacia patellae were compared with those in knees without chondromalacia patellae. For assessment of reproducibility, axial measurements were repeated by a second observer. RESULTS: There was no statistically significant difference in any of the axial and sagittal slice measurements between knees with and those without chondromalacia patellae. Interobserver reliability was excellent for measurements of trochlear depth and measurements in the superior and middle aspects of the patella. Measurements through the inferior patella were slightly less reproducible. CONCLUSION: The results of our study with MRI confirmed many previous radiographic findings. Although we did not find correlation between the presence of chondromalacia patellae and the patellofemoral indexes we analyzed, it is possible that the results of further investigations incorporating different grades of chondromalacia and different locations along the patellar articular surface may lead to further insight regarding the morphologic risk factors for chondromalacia patellae

Comparison of region-of-interest analysis with three different histogram analysis methods in the determination of perfusion metrics in patients with brain gliomas

PURPOSE: To compare routine ROI analysis and three different histogram analyses in the grading of glial neoplasms. The hypothesis is that histogram methods can provide a robust and objective technique for quantifying perfusion data in brain gliomas. Current region-of-interest (ROI)-based methods for the analysis of dynamic susceptibility contrast perfusion magnetic resonance imaging (DSC MRI) data are operator-dependent. MATERIALS AND METHODS: A total of 92 patients underwent conventional and DSC MRI. Multiple histogram metrics were obtained for cerebral blood flow (CBF), cerebral blood volume (CBV), and relative CBV (rCBV) maps using tumoral (T), peritumoral (P), and total tumoral (TT) analysis. Results were compared to histopathologic grades. Statistical analysis included Mann-Whitney (MW) tests, Spearman rank correlation coefficients, logistic regression, and McNemar tests. RESULTS: The maximum value of rCBV (rCBV(max)) showed highly significant correlation with glioma grade (r = 0.734, P < 0.001). The strongest histogram correlations (P < 0.0001) occurred with rCBV(T) SD (r = 0.718), rCBV(P) SD(25) (r = 0.724) and rCBV(TT) SD(50) (r = 0.685). Multiple rCBV(T), rCBV(P), and rCBV(TT) histogram metrics showed significant correlations. CBF and CBV histogram metrics were less strongly correlated with glioma grade than rCBV histogram metrics. CONCLUSION: Histogram analysis of perfusion MR provides prediction of glioma grade, with peritumoral metrics outperforming tumoral and total tumoral metrics. Diffusion-weighted MRI for quantification of liver fibrosis: preliminary experience

OBJECTIVE: The purpose of this study was to evaluate our preliminary experience using diffusion-weighted MRI for quantification of liver fibrosis. SUBJECTS AND METHODS: Diffusion-weighted MRI with single-shot echo-planar technique at b values of 50, 300, 500, 700, and 1,000 s/mm2 was prospectively performed on 23 patients with chronic hepatitis and on seven healthy volunteers. The apparent diffusion coefficient (ADC) was measured in four locations in the liver. Liver biopsy results (n = 19) were retrospectively reviewed by two hepatopathologists in consensus to determine stage of fibrosis and grade of inflammation. A Mann-Whitney test was used to compare the ADCs between patients classified with respect to having stage 2 or greater versus stage 1 or less fibrosis and stage 3 or greater versus stage or less 2 fibrosis. Receiver operating characteristics analysis was used to assess the performance of ADC in prediction of the presence of stage 2 or greater and stage 3 or greater fibrosis. RESULTS: Using a b value of 500 s/mm2 and all combined b values, we found significantly lower hepatic ADCs in stage 2 or greater versus stage 1 or less fibrosis and stage 3 or greater versus stage 2 or less fibrosis. The mean ADCs (x 10(-3) mm2/s) with all b values were 1.47 +/- 0.11 (SD) versus 1.65 +/- 0.10 for stage 2 or greater versus stage 1 or less fibrosis (p < 0.001) and 1.44 +/- 0.07 versus 1.66 +/- 0.10 for stage 3 or greater versus stage 2 or less fibrosis (p <0.001). Hepatic ADC was a significant predictor of stage 2 or greater and stage 3 or greater fibrosis, with areas under the curve of 0.896 and 0.896, sensitivity of 83.3% and 88.9%, and specificity of 83.3% and 80.0% (ADC with all b values, 1.54-1.53 x 10(-3) mm2/s or less). CONCLUSION: Diffusion-weighted MRI can be used for prediction of the presence of moderate and advanced liver fibrosis

Serial whole-brain N-acetylaspartate concentration in healthy young adults

Although the concentration of N-acetylaspartate (NAA) is often used as a neuronal integrity marker, its normal temporal variations are not well documented. To assess them over the 1-2 year periods of typical clinical trials, the whole-brain NAA concentration was measured longitudinally, over 4 years, in a cohort of healthy young adults. No significant change (adjusted for both sex and age) was measured either interpersonally or intrapersonally over the entire duration of the study

Quantitative assessment of iron accumulation in the deep gray matter of multiple sclerosis by magnetic field correlation imaging

BACKGROUND AND PURPOSE: Deposition of iron has been recognized recently as an important factor of pathophysiologic change including neurodegenerative processes in multiple sclerosis (MS). We propose that there is an excess accumulation of iron in the deep gray matter in patients with MS that can be measured with a newly developed quantitative MR technique--magnetic field correlation (MFC) imaging. MATERIALS AND METHODS: With a 3T MR system, we studied 17 patients with relapsing-remitting MS and 14 age-matched healthy control subjects. We acquired MFC imaging using an asymmetric single-shot echo-planar imaging sequence. Regions of interest were selected in both deep gray matter and white matter regions, and the mean MFC values were compared between patients and controls. We also correlated the MFC data with lesion load and neuropsychologic tests in the patients. RESULTS: MFC measured in the deep gray matter in patients with MS was significantly higher than that in the healthy controls (P < or = .03), with an average increase of 24% in the globus pallidus, 39.5% in the putamen, and 30.6% in the thalamus. The increased iron deposition measured with MFC in the deep gray matter in the patients correlated positively with the total number of MS lesions (thalamus: r = 0.61, P = .01; globus pallidus: r = 0.52, P = .02). A moderate but significant correlation between the MFC value in the deep gray matter and the neuropsychologic tests was also found. CONCLUSION: Quantitative measurements of iron content with MFC demonstrate increased accumulation of iron in the deep gray matter in patients with MS, which may be associated with the disrupted iron outflow pathway by lesions. Such abnormal accumulation of iron may contribute to neuropsychologic impairment and have implications for neurodegenerative processes in MS

Characterizing 'mild' in traumatic brain injury with proton MR spectroscopy in the thalamus: Initial findings

OBJECTIVE: Although most mild traumatic brain injury (mTBI) patients suffer any of several post-concussion symptoms suggestive of thalamic involvement, they rarely present with any MRI-visible pathology. The aim here, therefore, is to characterize their thalamic metabolite levels with proton MR spectroscopy (1H-MRS) compared with healthy controls. METHODS: T1-weighted MRI and multi-voxel 1H-MRS were acquired at 3 Tesla from 20 mTBI (Glasgow Coma Scale score of 15-13) patients, 19-59 years old, 0-7 years post-injury; and from 17 age and gender matched healthy controls. Mixed model regression was used to compare patients and controls with respect to the mean absolute N-acetylaspartate (NAA), choline (Cho) and creatine (Cr) levels within each thalamus. RESULTS: The mTBI-induced thalamic metabolite concentration changes were under +/- 13.0% for NAA, +/- 13.5% for Cr and +/- 18.8% for Cho relative to their corresponding concentrations in the controls: NAA: 10.08 +/- 0.30 (mean +/- standard error), Cr: 5.62 +/- 0.18 and Cho: 2.08 +/- 0.09 mM. These limits represent the minimal detectable differences between the two cohorts. CONCLUSION: The change in metabolic levels in the thalamus of patients who sustained clinically defined mTBI could be an instrumental characteristic of 'mildness'. 1H-MRS could, therefore, serve as an objective laboratory indicator for differentiating 'mild' from more severe categories of head-trauma, regardless of the presence or lack of current clinical symptoms

Signal characteristic and enhancement patterns of pancreatic adenocarcinoma: evaluation with dynamic gadolinium enhanced MRI

AIM: To determine the signal characteristics and enhancement patterns of proven pancreatic adenocarcinomas at 1.5 T and to compare these results with contrast enhanced computed tomography (CECT). MATERIALS AND METHODS: Twenty-five patients, mean age 73 years, with proven pancreatic adenocarcinoma were imaged at 1.5 T using in- and opposed-phase, gradient-echo (GRE), T1-weighted sequences, T2 weighting using either a short tau inversion recovery (STIR) or frequency selective, fat-suppressed turbo spin echo (TSE) sequence, and with a three-dimensional (3D), fat-suppressed, GRE T1 sequence before, during the arterial, venous, and equilibrium phases after Gadolinium administration. Fourteen of the 25 patients underwent CECT. Magnetic resonance imaging (MRI) examinations were evaluated by two observers in consensus for size, signal characteristics, and enhancement patterns, and the results were compared with CECT. RESULTS: The mean size of pancreatic adenocarcinomas was 32mm. On unenhanced T1-weighted images, 12 of 25 lesions (48%) were hypointense, 13 (52%) were isointense. On STIR/T2, 11 of 25 (44%) pancreatic adenocarcinomas were hyperintense, 14 (56%) were isointense. All 25 (100%) adenocarcinomas were hypointense during the arterial phase. Twenty (80%) and 17 (68%) remained hypointense in the venous phase and equilibrium phases, respectively. In seven of 14 (50%) cases, the pancreatic mass was iso-attenuating to the pancreatic parenchyma during both the pancreatic and venous phases of CECT. CONCLUSION: The results of the present study showed that all 25 pancreatic adenocarcinomas were hypointense to pancreatic parenchyma during the arterial phase. Moreover, MRI may be useful in patients with a high suspicion of pancreatic carcinoma that is not visualized during CECT

Renal cyst pseudoenhancement: influence of multidetector CT reconstruction algorithm and scanner type in phantom model

PURPOSE: To prospectively determine the dependence of renal cyst pseudoenhancement on multidetector computed tomographic (CT) scanner type and convolution kernel in a phantom model. MATERIALS AND METHODS: A customized anthropomorphic phantom was created to accept interchangeable 40-, 140-, and 240-HU renal inserts that contained stacked 0- and 50-HU cylindric cysts measuring 7, 10, and 15 mm in diameter. Each phantom and insert was scanned with five different multidetector CT scanners on five separate occasions by using 120 kVp, low and high tube current settings, 3.00-3.75-mm collimation, and standard and high-spatial-resolution kernels. A total of 2340 CT attenuation measurements were obtained by using standardized regions of interest. The effect of multidetector CT imaging regimen, tube current, cyst diameter, and renal attenuation on pseudoenhancement incidence was assessed by using generalized estimating equations based on a binary logistic regression model. Within this framework, a Bonferroni multiple comparison correction was used to assess pseudoenhancement frequency differences among imaging regimens. RESULTS: Pseudoenhancement occurred in both 0- and 50-HU cysts; was significantly correlated with multidetector CT imaging regimen (P<.0001), cyst diameter (P<.0001), and renal attenuation (P<or=.032); and was independent of tube current (P>.3). When convolution kernels on specific scanners were compared, significant differences (P<.04) between kernels were identified with all five scanners in terms of observed pseudoenhancement incidence. Generational differences in equipment were noted, with pseudoenhancement incidence ranging from 1.7% to 8.3%, 1.7% to 16.7%, and 18.3% to 56.7% across relevant kernels for three scanners from one manufacturer. CONCLUSION: Pseudoenhancement is strongly dependent on multidetector CT convolution kernel. Varying this parameter may mitigate this phenomenon, which is independent of volume-averaging effects

Muscle atrophy as a consequence of rotator cuff tears: should we compare the muscles of the rotator cuff with those of the deltoid?

PURPOSE: The quantitative assessment of muscle atrophy has a degree of importance in prognosticating rotator cuff treatment. However, it has been conjectured that muscle fat increases with aging. Therefore, we thought that the quantitative assessment of the supraspinatous would be better if made in comparison with a standard of reference such as the deltoid. Consequently, we performed a two-part study, first evaluating supraspinatous changes compared with the deltoid in 'normals' with aging, and second, determining if in patients with cuff tears the supraspinatous fat exceeds that of the deltoid. MATERIALS AND METHODS: In part 1, we studied 50 patients stratified by decade. In the first sitting, two blinded independent observers quantitatively graded the deltoid (with the supraspinatous obscured) and in the second sitting the same two observers quantitatively graded the supraspinatous (with the deltoid obscured). In part 2 of the study, we evaluated patients with moderate rotator cuff tears (>2 cm) and performed the same blinded, two-sitting, quantitative assessment (with the comparison muscle obscured). RESULTS: We found that muscle atrophy increases with age in patients without tears (0.011/0.028 U/year), although to a greater degree in the deltoid (p = 0.032). Also, in similarly aged patients, quantitative scores of the deltoid closely matched those of the supraspinatous (p = 0.071). Notably, however, in patients with large tears, the supraspinatous showed significant changes disproportionate to those of the deltoid, regardless of patient age (p = 0.044). CONCLUSION: In the presence of a normal rotator cuff, fatty infiltration increases with age. Age-related changes occur more frequently in the deltoid, verifying this muscle's potential as a standard of reference. With cuff tears, supraspinatous atrophy was disproportionate to that of the deltoid. Therefore, systematic assessment of supraspinatous muscle atrophy may be more reliable using the deltoid as a control for comparison than assessing it in isolation

Measuring elevated microvascular permeability and predicting hemorrhagic transformation in acute ischemic stroke using first-pass dynamic perfusion CT imaging

BACKGROUND AND PURPOSE: Hemorrhagic transformation (HT) can be a devastating complication of acute ischemic stroke (AIS). The purpose of this study was to determine whether increased microvascular permeability (PS) of the blood-brain barrier was detected in early AIS by using first-pass dynamic perfusion CT (PCT) and whether PS was significantly higher in infarcts destined for HT. MATERIALS AND METHODS: Fifty patients with AIS less than 3 hours old and evaluated by PCT were included. PS color maps were retrospectively generated from PCT data using the Patlak model. One reader analyzed each PS map by drawing 4 circular 10-mm regions of interest on any focal abnormality. The mean of these 4 regions of interest represented the PS of the infarct (PSinfarct). The mean of 4 mirror regions of interest on the nonischemic contralateral hemisphere was also obtained (PScontrol). PSinfarct and PScontrol were compared by using an exact Wilcoxon test. PSinfarct for infarcts that developed HT on follow-up (PSHT) was compared with all of the others (PSNo-HT) using an exact Mann-Whitney test. RESULTS: Forty-four infarcts (88%) showed focal PS elevation in the region of infarct. In units of milliliters per 100 milliliters per minute, PSinfarct ranged from 0 to 13 (mean: 3.5+/-3.1) versus PScontrol of 0-0.8 (mean: 0.28+/-0.27; P<.0001). Six infarcts (12%) developed HT, all of which were within the region of PS elevation. PSHT ranged from 5.2 to 13 (mean: 9.8+/-2.9) versus PSNo-HT of 0-5.9 (mean: 2.7+/-2.0; P<.0001). Eighteen infarcts (36%) were treated with recombinant tissue plasminogen activator (rtPA). A significant difference between PSHT and PSNo-HT persisted irrespective of rtPA treatment. CONCLUSIONS: Elevated permeability was detectable in AIS by using first-pass PCT and it predicted subsequent HT
