Focused Process Improvement Events: Sustainability of Impact on Process and Performance in an Academic Radiology Department

PURPOSE: To evaluate sustainability of impact of rapid, focused process improvement (PI) events on process and performance within an academic radiology department. METHODS: Our department conducted PI during 2011 and 2012 in CT, MRI, ultrasound, breast imaging, and research billing. PI entailed participation by all stakeholders, facilitation by the department chair, collection of baseline data, meetings during several weeks, definition of performance metrics, creation of an improvement plan, and prompt implementation. We explore common themes among PI events regarding initial impact and durability of changes. We also assess performance in each area pre-PI, immediately post-PI, and at the time of the current study. RESULTS: All PI events achieved an immediate improvement in performance metrics, often entailing both examination volumes and on-time performance. IT-based solutions, process standardization, and redefinition of staff responsibilities were often central in these changes, and participants consistently expressed improved internal leadership and problem-solving ability. Major environmental changes commonly occurred after PI, including a natural disaster with equipment loss, a change in location or services offered, and new enterprise-wide electronic medical record system incorporating new billing and radiology informatics systems, requiring flexibility in the PI implementation plan. Only one PI team conducted regular post-PI follow-up meetings. Sustained improvement was frequently, but not universally, observed: in the long-term following initial PI, measures of examination volume showed continued progressive improvements, whereas measures of operational efficiency remained stable or occasionally declined. CONCLUSIONS: Focused PI is generally effective in achieving performance improvement, although a changing environment influences the sustainability of impact. Thus, continued process evaluation and ongoing workflow modifications are warranted.

Association between changes in suspicious prostate lesions on serial MRI examinations and follow-up biopsy results

We assessed changes in prostate lesions on serial magnetic resonance imaging (MRI) examinations in predicting biopsy results. Fifty-five men undergoing two prostate MRI examinations >/=6months apart, followed by targeted biopsy, were included. Two radiologists assessed dominant lesions for an increase in size or suspicion score. Progression on MRI had lower sensitivity (23.5%-35.3%) and higher specificity (76.2%-90.5%) than prostate-specific antigen (PSA) velocity (sensitivity 70.6%, specificity 52.4%) for predicting positive biopsy. Highest accuracy was achieved by PSA velocity (63.6%) for positive biopsy, and by MRI (65.5%-72.7%) for Gleason >6 tumor. Findings support lesion progression on MRI serving as a basis for performing subsequent targeted biopsy.

Differentiation of Malignant Omental Caking from Benign Omental Thickening using MRI

PURPOSE: To determine multi-parametric MRI features that can help differentiate malignant omental caking from benign omental thickening in the setting of portal hypertension. METHODS: We identified 19 patients with an abnormal omentum on MRI and an available reference standard: 11 patients with portal hypertension and benign omental thickening (9 male, 2 female, mean age 58 +/- 6 years) and 8 patients with metastatic omental caking (4 male, 4 female, mean age 61 +/- 13 years). Criteria for benign omental thickening were no evidence of malignancy for at least 24 months of follow-up (n = 7), negative ascites cytology (n = 2), or absence of malignancy on pathologic analysis of liver explant (n = 2). Criteria for omental malignancy were positive omental biopsy (n = 6) or ascites cytology (n = 2). Two radiologists (R1 and R2) evaluated characteristics of the thickened omentum on MRI. RESULTS: Findings occurring with significantly higher frequency in malignant omental caking were hyperintensity on high b-value diffusion-weighted imaging (DWI) (R1 88% vs. 0%, R2 88% vs. 0%), hyperenhancement (R1 75% vs. 0%, R2 75% vs. 0%), and convex outer omental contour (R1 88% vs. 0%, R2 75% vs. 9%) (all p /= 0.058). CONCLUSION: Abnormal signal on DWI, hyperenhancement, and convex outer contour are helpful MRI features to differentiate malignant from benign omental thickening.

New OPTN/UNOS Classification System for Nodules in Cirrhotic Livers Detected with MR Imaging: Effect on Hepatocellular Carcinoma Detection and Transplantation Allocation

Purpose To assess the effect of the new Organ Procurement and Transplantation Network ( OPTN Organ Procurement and Transplantation Network )/United Network for Organ Sharing ( UNOS United Network for Organ Sharing ) policy on hepatocellular carcinoma ( HCC hepatocellular carcinoma ) detection and liver transplant allocation in patients with cirrhosis undergoing dynamic contrast material-enhanced liver magnetic resonance (MR) imaging. Materials and Methods In this HIPAA-compliant institutional review board-approved retrospective study with waiver of informed consent, 247 patients (196 men, 51 women; mean age, 60 years +/- 11 [standard deviation]) with liver cirrhosis who underwent evaluation for HCC hepatocellular carcinoma with MR imaging were identified via database search. Three radiologists independently reviewed images and identified number and size of HCC hepatocellular carcinoma based on criteria within either the prior or revised policy. Based on these interpretations, priority for liver transplantation for each patient was determined with prior and revised transplantation allocation criteria. HCC hepatocellular carcinoma detection was compared between sessions by using McNemar tests, and interreader agreement for detection of at least one HCC hepatocellular carcinoma was assessed by using kappa coefficients. T2-weighted imaging of the prostate: Impact of the BLADE technique on image quality and tumor assessment

PURPOSE: To retrospectively compare standard and BLADE T2-weighted imaging (T2WI) sequences of the prostate in terms of image quality and tumor assessment. METHODS: 49 prostate cancer patients (64 +/- 6 years) who underwent 3 T phased-array coil MRI before prostatectomy were included. T2WI was acquired using standard rectilinear and BLADE techniques. Two readers (R1, R2) independently localized the dominant lesion using T2WI alone and using multi-parametric imaging; recorded presence of extraprostatic extension (EPE) in each lobe; and scored lesion conspicuity and absence of motion artifact (1-5 scale; 5 = highest quality). A third reader, unblinded to pathology, placed ROIs to record tumor-to-peripheral-zone contrast. Standard and BLADE T2WI were compared using paired Wilcoxon tests. RESULTS: BLADE showed a trend toward improved motion artifact for R1 (3.4 +/- 1.3 vs. 2.9 +/- 1.5; p = 0.054) but not R2 (4.0 +/- 1.0 vs. 3.9 +/- 1.1; p = 0.880). Dominant lesions showed significantly lower conspicuity using BLADE for R1 (2.8 +/- 2.0 vs. 3.2 +/- 2.0; p = 0.011) but not R2 (2.3 +/- 1.6 vs. 2.4 +/- 1.7; p = 0.353), and significantly lower tumor-to-peripheral-zone contrast using BLADE (0.35 +/- 0.13 vs. 0.42 +/- 0.15; p

Whole-lesion diffusion metrics for assessment of bladder cancer aggressiveness

PURPOSE: To explore associations of whole-lesion histogram diffusion metrics with pathologic findings and subsequent metastatic disease in bladder cancer patients undergoing radical cystectomy. METHODS: Twenty-three bladder cancer patients (21M, 2F; mean 70 +/- 11 years) underwent MRI before cystectomy. A volume-of-interest was placed on all slices on the ADC map encompassing each lesion. Whole-lesion mean, kurtosis, and skewness of ADC were calculated and compared with T stage and pelvic nodal status at cystectomy and with subsequent metastasis in 20/25 patients with available follow-up. RESULTS: At cystectomy, 39 % (9/23) were stage T2, 61 % (14/23) >/=T3, and 28 % (5/23) exhibited positive nodes; 35 % (7/20) developed later metastases. Mean ADC was significantly lower in stage >/=T3 than in lower stage tumors (1.20 +/- 0.36 x 10-3 vs. 1.55 +/- 0.36 x 10-3 mm2/s; p = 0.044), but showed no association with nodal or metastatic disease (p = 0.362-0.709). Kurtosis was significantly lower in tumors with, compared to without, nodal disease (-0.05 +/- 0.29 vs. 0.91 +/- 1.16; p = 0.037), and showed a non-significant decrease in tumors with, compared to without, later metastases (0.23 +/- 0.63 vs. 0.83 +/- 0.89; p = 0.088). Kurtosis was not associated with T stage (p = 0.811), and skew was not associated with any outcome (p = 0.516-0.643). Mean ADC achieved highest AUC for identification of stage >/=T3 (AUC = 0.754 vs. 0.516-0.643 for other metrics). Kurtosis achieved highest AUC for nodal disease (AUC = 0.811 vs. 0.522-0.556 for other metrics) and metastases (AUC = 0.736 vs. 0.516-0.626 for other metrics). Only difference in AUC between skewness and kurtosis for nodal disease was significant (p = 0.031). CONCLUSION: While requiring larger studies, kurtosis has potential to complement mean ADC in bladder cancer prognosis using whole-lesion histogram analysis.

Diffusion-weighted imaging of the liver: comparison of image quality between monopolar and bipolar acquisition schemes at 3T

PURPOSE: To compare image quality of monopolar and bipolar diffusion-weighted imaging (DWI) sequences of the liver at 3T. METHODS: 32 healthy volunteers (mean 27 +/- 8 years; 27 M/5F) and 11 patients (mean age 58 +/- 14 years; 8 M/3F) underwent liver MRI using a 3T system incorporating 2-channel parallel transmission for B1-shimming and reduced B1-inhomogeneity. Scans included free-breathing DWI sequences (b-value 0, 400, 800 s/mm2) acquired using both monopolar and bipolar techniques. Estimated signal-to-noise ratio (eSNR), apparent diffusion coefficient (ADC), and measures of subjective image quality on b-800 images, scored on a 1-5 scale by two independent radiologists, were compared between sequences. RESULTS: Monopolar sequence demonstrated significantly higher eSNR (volunteers: 12.7 +/- 4.0 vs. 11.3 +/- 3.5, patients: 11.4 +/- 4.0 vs. 10.2 +/- 3.3; p /= 0.191). Hepatic ADC was significantly lower using monopolar technique only in volunteers (1.28 +/- 0.12 vs. 1.43 +/- 0.15, p < 0.001). CONCLUSION: In comparison with past studies performed at 1.5T, when using a modern 3T system, we observed improved image quality of liver DWI using a monopolar, rather than a bipolar, acquisition scheme, largely attributed to higher eSNR.

Zoomed echo-planar imaging using parallel transmission: impact on image quality of diffusion-weighted imaging of the prostate at 3T

PURPOSE: To assess impact of two-channel parallel transmission (pTx) with focused excitation [zoomed echo-planar imaging (EPI)] on image quality of prostate diffusion-weighted imaging (DWI) at 3T. METHODS: 27 male volunteers (27 +/- 8 years) underwent 3T prostate MRI using 2-channel radiofrequency-transmit system and 18-channel torso receive coil. Scans included EPI-DWI sequence (b values 50, 500, 1000 s/mm2) acquired both with standard sinc pulse and 2-channel pTX with focused excitation, each acquired at large-field-of-view (FOV) (20 x 20 cm) and small-FOV (14 x 14 cm). An abdominal radiologist scored b-1000 images and ADC maps for image quality measures. Sequences were compared using paired Wilcoxon tests. RESULTS: pTx with focused excitation showed significant improvements compared with standard DWI on b-1000 images at large-FOV for the absence of wrap and overall image quality (p /= 0.175), while showing significant improvements on the ADC maps in terms of reduced distortion, absence of ghosting, and absence of wrap (p = 0.010-0.030). CONCLUSION: Zoomed DWI using 2-channel pTx reduced artifacts and improved image quality for 3T prostate DWI; benefit was most apparent for small-FOV images.

Dynamic contrast-enhanced MRI of the prostate with high spatiotemporal resolution using compressed sensing, parallel imaging, and continuous golden-angle radial sampling: Preliminary experience

PURPOSE: To demonstrate dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI) of the prostate with both high spatial and temporal resolution via a combination of golden-angle radial k-space sampling, compressed sensing, and parallel-imaging reconstruction (GRASP), and to compare image quality and lesion depiction between GRASP and conventional DCE in prostate cancer patients. MATERIALS AND METHODS: Twenty prostate cancer patients underwent two 3T prostate MRI examinations on separate dates, one using standard DCE (spatial resolution 3.0 x 1.9 x 1.9 mm, temporal resolution 5.5 sec) and the other using GRASP (spatial resolution 3.0 x 1.1 x 1.1 mm, temporal resolution 2.3 sec). Two radiologists assessed measures of image quality and dominant lesion size. The experienced reader recorded differences in contrast arrival times between the dominant lesion and benign prostate. RESULTS: Compared with standard DCE, GRASP demonstrated significantly better clarity of the capsule, peripheral/transition zone boundary, urethra, and periprostatic vessels; image sharpness; and lesion conspicuity for both readers (P < 0.001-0.020). GRASP showed improved interreader correlation for lesion size (GRASP: r = 0.691-0.824, standard: r = 0.495-0.542). In 8/20 cases, only GRASP showed earlier contrast arrival in tumor than benign; in no case did only standard DCE show earlier contrast arrival in tumor. CONCLUSION: High spatiotemporal resolution prostate DCE is possible with GRASP, which has the potential to improve image quality and lesion depiction as compared with standard DCE.J. Magn. Reson. Whole-lesion apparent diffusion coefficient metrics as a marker of percentage Gleason 4 component within Gleason 7 prostate cancer at radical prostatectomy

PURPOSE: To retrospectively assess the utility of whole-lesion apparent diffusion coefficient (ADC) metrics in characterizing the Gleason 4 component of Gleason 7 prostate cancer (PCa) at radical prostatectomy. MATERIALS AND METHODS: Seventy patients underwent phased-array coil 3T-magnetic resonance imaging (MRI) before prostatectomy. A uropathologist mapped locations and Gleason 4 percentage (G4%) of Gleason 7 tumors. Two radiologists independently reviewed ADC maps, aware of tumor locations but not G4%, and placed a volume-of-interest (VOI) on all slices including each lesion on the ADC map to obtain whole-lesion mean ADC and ADC entropy. Entropy reflects textural variation and increases with greater macroscopic heterogeneity. Performance for characterizing Gleason 7 tumors was assessed with mixed-model analysis of variance (ANOVA) and logistic regression. RESULTS: Among 84 Gleason 7 tumors (G4% 5%-85%, median 30%; 59 Gleason 3+4, 25 Gleason 4+3), ADC entropy was significantly higher in Gleason 4+3 than Gleason 3+4 tumors (R1: 5.27 +/- 0.61 vs. 4.62 +/- 0.78, P = 0.001; R2: 5.91 +/- 0.32 vs. 5.57 +/- 0.56, P = 0.004); mean ADC was not significantly different between these groups (R1: 0.90 +/- 0.15*10-3 cm2 /s vs. 0.98 +/- 0.21*10-3 cm2 /s, P = 0.075; R2: 1.06 +/- 0.19*10-3 cm2 /s vs. 1.14 +/- 0.16*10-3 cm2 /s, P = 0.083). The area under the receiver operating characteristic (ROC) curve (AUC) for differentiating groups was significantly higher with ADC entropy than mean ADC for one observer (R1: 0.74 vs. 0.57, P = 0.027; R2: 0.69 vs. 0.61, P = 0.329). For R1, correlation with G4% was moderate for ADC entropy (r = 0.45) and weak for mean ADC (r = -0.25). For R2, correlation with G4% was moderate for ADC entropy (r = 0.41) and mean ADC (r = -0.32). For both readers, ADC entropy (P = 0.028-0.003), but not mean ADC (P = 0.384-0.854), was a significant independent predictor of G4%. CONCLUSION: Whole-lesion ADC entropy outperformed mean ADC in characterizing Gleason 7 tumors and may help refine prognosis for this heterogeneous PCa subset. J. Magn. Reson. Imaging 2014.
