Reviews the book, Dialectical behavior therapy with suicidal adolescents by A.L. Miller and J.H. Rathus (see record 2006-23301-000). This book is as much a reflection of the careers of Miller and Rathus as it is a synthesis of practical and theoretical advice for dealing with the suicidal adolescent. The book is rooted in the current evidence based literature on dialectical behavioral therapy and suicidology. It also acts as a workbook, challenging the reader to interpret situations with a dialectic framework. The book is divided into 12 chapters and 3 appendices. This book was immediately useful in working with adolescents on an inpatient unit at a major city hospital, but its use clearly can be extrapolated to a variety of clinical situations, in particular to clinicians who work with suicidal patients and their families. The book reflects its commitment to dialects in the way it walks the middle path between the practical and theoretical.

Capacity assessment and intervention in 56-yr-old man with schizophrenia [Case Report]

The importance of the doctor-patient relationship among patients with schizophrenia is especially evident within the context of medication adherence. The association between patients' positive perception of their psychiatrist and medication adherence has been established in several studies. This is the case of a 56-year-old man with schizophrenia and multiple medical problems who is refusing to take any medications. We discuss the risks and benefits of having a patient declared incompetent along with assessing the patient's capacity. The case highlights situations when a physician might question a patient's decisional capacity but conclude that the decision should be accepted so as to maintain rapport with the patient and continue treatment

Alzheimer's disease. Accurate and early diagnosis in the primary care setting

Alzheimer's disease is the most common dementia type and is characterized by a gradual, progressive decline in multiple areas of cognition and function. Early diagnosis is key because it can initiate the process of patients and family adapting to and managing disease symptoms. Moreover, certain pharmacologic interventions can impede symptom progression and significantly improve quality of life. A spectrum of basic tests and instruments make clinical diagnosis of AD attainable in the primary care setting. Treatment with cholinesterase inhibitors is targeted toward cognitive enhancement.
